CLUB FEDERATION ANNUAL DUES

(Dues Only)

Annual dues of $20.00 for the South Carolina Square and Round Dance Federation for 2008 are due by November 15, 2007. Complete the attached Membership roster and return three (3) copies with check to address indicated below.

CLUB ANNUAL DUES AND INSURANCE

If your club desires Insurance through the S.C. Square and Round Dance Federation, the premium is due by November 15, 2007. The amount is calculated by the instructions on the Insurance Information Form. This form must be completed and sent with check. Also complete and return three (3) copies of the attached membership roster.

Your Treasurer can write one check to cover both fees, but it must be received by November 15, 2007. The check for dues and/or insurance premium should be made out to South Carolina Square and Round Dance Federation.
